Just airing out an idea that came to mind today.

A young guy who previous had an inguinal hernia decides to go in to get checked, after experiencing some discomfort in the area. His insurance has changed since last time, and so goes to a new provider. He's sitting in the office waiting to be examined by the physician's assistant. The door opens, and lo and behold, it's the attractive, slightly older (early 30s) woman that attends his church. He vaguely remembers knowing she was a PA, but definitely didn't know she worked here...until now.

Many possibilities:

1) Is this the rule or the exception for the lady? Does she usually not examine male patients like this? Or does she, but this is the first time with someone she knows?

2) What gets the pot stirring? Is the young man well-endowed? Or if not, does the mere sight, combined with knowing they would meet again that very Sunday, stir up feelings of lust in her?

3) How do they view the situation? Normally, a devout married woman would definitely not be seeing or handling genitalia of another man. But circumstances are different here....

4) Did they know each other well before? A small acquaintanceship? Or is this the first time they've spoken at length?

That being said...

Erotica starting with a medical exam are typically full-blown “medical fetish” or “naughty women in uniforms and positions of authority” stories. So realism tends to take a back seat. But it sounds like you’re looking for some psychological and social verisimilitude.

Unfortunately, I can’t help you there.

So the new health provider is performing mind and body altering satyrism and nymphomania clinical trials but the MILF from church has been afflicted via accidental contagion with the Handwavium and has become a devious, pervy sex-mad psychopath who has a Plan to feed their Church’s congregation’s entire over-18 population* vast amounts of the Handwavium just to see what would happen.

I think this would be a slightly more believable approach, plenty plausible. A good slow burn to sex is never a bad thing.
