What happens when you get your nipples pierced? Do they give you an anaesthetic? Do they use a punch? (I picture something like a paper punch) Does it hurt a lot?

I think they use ice. That would sure make them stand out for the paper punch.

Needle, not a punch.

I believe some places do offer anesthetic, but not positive on that one.

had mine done, but....

no anesthetic.. just a clamp and a hollow needle.. it was quick and yes, it hurt but not badly...
however,
if you are a water rat like i am.. i would suggest waiting until fall to have it done.. i had mine done in june two years ago, swam all summer and wound up having surgery.. lost an entire cup size..

i would do it again.. but i think im a bit chicken now..

From what I know of piercers, they'd give you a topical anesthetic if you begged and pleaded and whined. However, while the pain is excruciating, it is temporary and followed by a very divine warmth, in my experience. Do it. I've never heard of someone regretting them, actually.
