I have been playing PnP RPGs for quite a long time now. Several of our game had some pretty adult themes. For example: in High Colonies our team traveled around in a ship that was a Casino, small cruise ship, and a Bordello. This covered our movements. Just about any good game can be made into an adult game.
My favorite system the Talislanta. It is a unique fantasy setting that still advertises "No elves!" This game lends itself well to adult themes. From the hedonistic city of Zandu to the magical city of Cymril there are plenty of locales to set a hot campaign in. A lot of the material has some adult overtones, with slavery happening in many parts of the continent as one example. Another was the mention of a certain Cymrillian wizard who had the habit of changing his appearance and even his(?) gender on a whim with certain magical enhancements in one of the books. You can play as a Batrean Concubine who has the power to beguile men with her scent.
If mechanics were any worry, the system is easy to get into, it pretty simple, yet it is very flexible and powerful. Most of the stuff in the books is background on the world. It gives you a big "sandbox" to play in, yet it doesn't constrain you to an official storyline.
One of the best things is that the creator of the game has released the entire library--minus some non-canon books--for free download. A lot of the fans have been working hard to scan and post all of these books. This is a labor of love for the fans. All of this is at the talislanta.com site.
