I have several friends who specialize in using pain to pleasure a woman. One of them loves to have a cheating wife invite him to her home when her hubby is gone. He instructs her to strip down to her panties. Then he has her pirouette and and then tie her spread-eagle on her marriage bed. to use a sharp knife to cut the crotch out of a woman’s panties. He is ever so careful to just nick her clit while her does. Not really much blood, but he makes sure to use a ball gag to muffle the scream. He revels in watching her after the nick, because, he says that after the initial scream the woman's clitoris throbs, and most of the women have continuous orgasms without being touched. His record is 93 minutes before the inevitable loss of consciousness.

Is this beyond the pale as a subject for a Literotica story?

the rules are not clear, but I suspect this falls on the wrong side of the "physical harm" line.

He cuts a cheating wife's clit, no matter how slightly, and they pass out over time? And you're asking if this is sexy or too extreme for Lit?

Maybe not prior to 2002 when they had their Extreme link (and sister site).

It's absolutely going to be over the line for some people, but not for others. On the whole most people recognise that other people's kinks are their own and as long as you're flagging your content properly I think most readers are likely to leave you to it. Ironically, it's the "punish a cheating wife" crowd who are most intolerant on that score. For that reason, if you're writing with those themes you might find some hostility on the forums, simply because your fanbase is just so toxic.

There isn't an explicit ban on stories featuring sexual violence, including rape or torture - there are examples of things like branding and knifeplay in the BDSM section, so actual bodily harm isn't automatically verboten - but they can definitely be rejected on the basis of that kind of content. From what I gather, the deciding factor seems to be whether the subject ends up enjoying the experience.

A story where a woman is subjected to genital mutilation and has an orgasm until she passes out might make it through, if it's presented as a positive experience for her. A story where the exact same act is presented as a righteous punishment for her adultery probably won't.
