I envision this idea as a bondage tale (i.e. the porn star is a bondage porn star) but it would work just as well as something else.
A gentleman sees a girl who looks exactly like his favorite porn star. This colors his interpretation of her actions. Her innocence is interpretted as an act, her hesitance towards various sexual acts is given less credence than it should otherwise. He does eventually have sex with her. Everything he pushes her into doing, she ends up enjoying. By the time he is done pushing, she is quite willing...and very thankful to have had her barriers removed. Much to his surprise, he discovers that a birthmark he had noted on his lover (one which could not be seen without nudity) is missing from the porn star. Perhaps this realization occurs after or during sex with the lover. Mayhaps it happens when he masturbates to the porn star's picture.
One could go with an unreliable narrator and not reveal she is not a porn star until the end.
