Editing a Title along with the story

I have a story edit in progress, and it will affect the title as well. My question is, when I submit an edit, how do I go about mentioning that I want the title changed too?

I feel like if I put in the new title along with EDIT, that new title will be what is searched for to replace, and won't be found (at least not in an automated sense).

Should I put the title change request into the Note to Admin box at the bottom?
