I like it. The patient could be someone who had a medical background - parents were doctors, or she dropped out of medical school - enough so that she could actually fake being a doctor. Patient and doctor look enough alike where patient could take over doctor's identity and escape the psychiatric facility leaving real doctor locked up, diagnosed with schizophrenia/personality disorder.
