Personally, I never saw the benefit of the Simultaneous O. Serial orgasms allow you to concentrate on the pleasure you are giving, or receiving, without any distractions!

However, to address your question, here is what I would suggest (not that I think it would work, but it could make a hot story): They decide to take each other to the brink, over and over. She gets him just about to come, then backs off. Then he does the same for her. The him, then her. Different ways each time, of course (oral, manual, etc.), to keep the story interesting. Finally, when they have both reached a feverish state of hair-trigger-ism, he mounts her and they both BLAST OFF.
