That's a very good question. I'm not sure how that would work, but there could be some interesting interactions. Now, you have my attention. I'm going to give this a some thought. I might even write a story along this line. Thank you for the feedback.
In your story it was stated that neither the main character, nor his wife were interested in direct interaction with other people, looking is ok, but no sex. And in their first experience with CFNM it was just that - the third person didn't touch anybody, didn't give any commands, there was nothing coming from her side at all.
This doesn't look like an intentional CFNM to me. More like they are a couple that lives together but has very different work schedules. She is ... I don't know... a nurse, comes home after a night shift, and finds her partner in bed with morning wood. So, she takes care of it. He is naked because that's the way he sleeps, she is dressed because she just came in, neither of them is treating this as a D/s scene.
You pose some very good questions here. Initially, I think the ladies would control their own husbands. My late wife was not into sharing.
