I have heard of a hand job technique, which I am sure could be adjusted to include oral. Basically you put your palms on the sides of the man's penis and then intertwine your fingers around his shaft. You could then suck the top of his penis (while giving a hand job) or you could lick the underside of his cock while using the backet to hold him in place.
