So I've got a rough idea of how the story should look:

Hero signs up for an escape room with his best friend, but not much information is provided beforehand. The online reviews sound generic, all saying that it’s a must-try and a ton of fun.

He is incapacitated and placed in a room with a woman. There's some kind of device in the room that has a short description how it is operated.

I wanted to start with more innocent tasks - some kind of milking device for the hero. The condition for the door to open is that he has to cum. The next room would have some toy for the woman. The further the players progress, the more interesting the challenges will become to advance in the game – of course, this will involve them in all sorts of exciting situations.

I'm still not sure about how the machines would look or how they'd be operated. Perhaps fingerprint sensors, so that players can only operate the machine in one specific way? How would I go about making sure that the participants have an orgasm? A heart rate monitor would be unreliable, right?

Red 1: 100 orgasms for the audience and at least 3 in any combination from the players.

Each doubling opens the next tier - ie, 50 orgasms in Yellow 1 opens the Red 2 door, 100 opens Red 2 and Yellow 2, and 200 opens all three doors (Red 2, Yellow 2, Green 2)

The (demonic?) Escape Room hosts could also have rules like, "Neither escapee can touch the penis or vulva in this room," so he has to use a prostate massager and she the nipple vibrators and vibrating butt plug, unless they're clever enough to understand the technicality that they could just put on the rubber gloves also in the room (but in a less obvious place). There are no condoms, so no sex in that room.

This could actually be a reasonable outline for a story game.

I would suggest starting a little slower - not all challenges need to be solved through orgasm.

Maybe the first challenge could be to fill up a basket with items - requiring the participants to get mostly undressed to get there.

Another could be some version of Twister, creating plenty of skin on skin without being sexual as such.

Yet another could be to answer sexually themed riddles - maybe exposing some of their own desires in the process.

Also, there could be challenges related to inserting large objects in orifices, lasting a certain number of minutes with vibrators etc.

Nice idea and lots of options - I would read it :)

Try to avoid fleshing out a huge rule book. Keep it simple to avoid continuity problems.

This has the potential to turn into a "Saw" type story with the controller watching through cameras. I think it's probably the only realistic way to do it to avoid cheating.

Make the MMCs self proclaimed alpha males and have the final puzzle result in them having to bottom for a sissies that look like their girlfriends.
 
Potential backstory - an AI was set to researching porn and human sexuality as reflected in porn's funhouse mirror view. The AI tapped into unexpected resources and is 'waking up,' and has at least partially escaped containment/control by the original researchers.

IT is the watcher, and it's learning as it goes. Some challenges may be too harsh, or too easy - but it'll learn, given time.

(How it makes all these rooms and contraptions, I leave as an exercise for the clever. Fiverr perhaps? The funding becomes a question...)

That being said - LoneMilf's point is well put, IMO. Maybe a fairly loosely woven wire basket on a bouncy pillar with a sign, "Fill the basket". The room is otherwise empty. The players can push down on it, and see the lights come on, but if they remain in contact with it, buzzing can be heard. 15 seconds after the buzzing, a nasty but non-fatal taser shock through the basket happens and repeats every 15 seconds.

Zfrk's correct, if rules carry over it could become a mess quickly. If, however, each room is self-contained, that eases things signficantly, IMO. Each room then has to have a challenge of some sort and one or more doors leading out which unlock depending on how much the players complete within that one room. Or at least that's my current thinking!
Note that the sign is enough for one light upgrade by itself, if they think to take it off its mounting!

The 'Twister' version could be "connect as many pads as possible", with rows of pads 1' apart across the floor. Spread-eagle stretch on the floor can cover enough to get red, if they touch hands and do so they can get yellow. However, it's number of connections - so touch and release and touch again counts as 2 connections. Maybe countdown displays over the doors showing the total numbers of contacts required to unlock each, dropping 1 by 1 as connections are made?

For a specific reason later in the story, my original thought was to make the game playable without a game master at all, using only sensors or similar devices to unlock the next room. But since we already have a live stream, why not let the viewers decide whether they agree if the task is complete?

The idea of having more than one exit conflicts with a plot twist that’s planned for later. I hadn't intended for individual tasks to be cheat-able or simplified. After all, the audience should be offered something exciting, right?

Compare it to a "reality TV" type show. The structure of those is usually challenges, which earn the players something. Can be points, can be actual dollars to take home, can be physical prizes. If the players are competitors (who have to cooperate to escape), you could get some real tension going.

Everyone is worried about the guy having to cum too often. Maybe that's the challenge sometimes. He's in a room that he must escape. Perhaps there's a woman in a Czech glory hole. He escapes if he makes her cum. I like the idea of escalating tension. Maybe the increasing tasks culminate in the final room where you cum. Or, increasing tension, put in more sexual situations with your partner and you LOSE if you cum before making it out the final room.

So, maybe the first task is like the Halloween game where you pass apples without using your hands... only held under your chin. Then maybe something similar, but with your mouth, requiring you to transfer something with your tongue... that your partner licks or sucks off.

The next room is pitch dark. They fumbling around in the dark cluttered tight quarters, holding each other for stability. Soft light bits of cloth float down from above - some glowing in the dark, some invisible in the darkness. The bits of cloth stick to their clothing with static electricity.
