I would first of all go with a man well above 300 lbs. Unable to feel his own penis, and haven't had sex for ages, he undergoes a new experimental gastric bypass surgery that not only gets rid of all the weight but he ends up going to the other extreme turning to skin and bones. Now he's too weak to have sex.
A second idea would to have the fat sexless guy go out with a skinny girl who loves him for who he is. One night he rolls during sex and she's crushed to death. The follow up courtroom drama say that their love was a sham because he was so huge.

How about, the guy is only chubby at the beginning but he's really shy, and his older brother (rich powerful executive who is also big but in more of a muscular football player way) hires a consultant (mage if a historical fantasy twist is allowed) who specializes in giving guys makeovers to make them more attractive. He however (since I'd rather read a gay story, but substitute a woman if you like) turns out to be a daddy-dom type and a chubby chaser. He falls for the shy guy and begins seducing him and also makes sure to feed him various deserts and things between kisses to help him get over the idea society has beaten into him that fat is disgusting. The shy man learns about being a submissive and particularly enjoys being tied up, but is worried over the fact that he's gaining weight and has a panic attack when he breaks 300 lbs. The dom helps shy guy overcome this buy giving the chubby the guy the idea that fat is like natural restraints that are totally inescapable because he has to wear them 24/7. So finally chubby guy sees his weight in a positive and erotic light and becomes an eager gainer, with his dom's encouragement and compliments gaining another hundred pounds or so. (Being a romance writer at heart I'm tempted to throw in a wedding and a baby here, but whatever.)
