It was done at the famous 252 Tattoo in Olmsted Township, OH
The tattoo artist was Dave, he's one of the best I've ever met. The tattoo was $170 before tax.
The tat is from the artist Alex Grey who did a ton of this type of artwork for Tool's last album Lateralus.
Here is a image of the tattoo, I took a picture of my T-shirt and printed this for Dave.

And here is a photo I had Dave take of my bicep halfway through the tattoo process.

I don't have a photo of the finished product yet, but it looks exactly like the artwork off the T.
It's healing now pretty well. The flaking has started and my skin around the tat is red-hot.
I have two other tat's one of a Joker and one of the Comedy/Drama masks.
