It is supposed to be a one size fits all device. It has slits on the top and bottom for air circulation. It also has a bit cut out at the end about an inch and half long and about a 1/4" wide to urinate through (which was designed well enough for most size 'states'). But given all that it wasn't always simple to piss through the slit. i had to make sure that everything was lined up properly for example - also waking up in the morning while still erect and needing to go was very challenging.

There were some instructions i found on the internet about how to put on and keep clean - it suggested using a flannel and soapy water. i also managed to push a tissue through the end and then use something to wipe around the inside edges (however it stayed relatively clean so this wasn't needed that regularly).
