More and more insurers are finding new and inventive ways not to pay claims and force the cost back onto the people who have already shelled out tens of thousands of dollars. Here's the newest trick: if you go to the emergency room for what the insurer considers something minor or was not needed right away to avoid a serious risk to health, the tab is on you.

I didn't bother posting the story of the woman who needs a liver transplant (she has a medical condition) and a whole team of doctors all agreed without the transplant she will die.

The insurer said, "No, we don't think so" and will not pay for the procedure.
