I'm sorry - just saw your question now.
It depends on the program. There are of course some default settings, where you can't control much, and in others you can control a lot of the specifics regarding the lighting; size and type of source, color temperature, shadow hardness and brightness etc. In fact, in Daz Studio (the program I usually use) I don't even understand all the controls - some are very technical and totally over my head.
But as a photographer (my "day job") what I enjoy about using the program is that the digital lights work very much like real lights do, so the manner in which I place them is very similar to how I would work if lighting an actual photography assignment.
