There are details in suckonsimon's excellent thread, but in essence, you have to be completely unaroused when putting the ring on. Push one ball inside the ring, then ease the second in by stretching the scrotum through the ring. Finally you tuck your cock through the remaining gap in the ring - this is why you have to be flaccid!
Oh, and never try to remove the ring if you're still even slightly tumescent, or you might injure your delicate parts...
